OKLAHOMA CITY – A Wyandotte girl who died in a playground incident last year suffered a severe concussion, the Oklahoma Medical Examiner's office said Tuesday.
Alyssa Avila died last August when she fell off a piece of playground equipment called the X-Wave, a teeter-totter like contraption set end to end, linked with flexible material.

The Oklahoma Medical Examiner said the nine-year-old died from "Commotio Cerebri," or trauma which caused a severe concussion. The ME's office ruled her death accidental.
Several school districts removed the X-Wave following the incident.
